Complete your preventive screenings and get Vitality Points
The first Synod-wide Challenge of 2025 is the “Prevention Challenge,” and here’s what you need to know.
Dates of Challenge: Now through March 31
Details:
- Complete at least one of the preventive health screenings you may have due recommended by your primary provider (e.g., mammogram, colonoscopy, eye/dental exam, annual physical, pap smear).
- In addition to the Vitality Points you’ll already earn for completing preventive exams and submitting proof within Vitality, if you complete your screening exam within the Challenge dates (Jan. 1-March 31) AND submit proof by Friday, April 4, you’ll earn an additional 250 Bonus Vitality Points.
- Since these activities do not automatically flow into Vitality, here’s where you’ll need to submit your preventive and health management activity for points at powerofvitality.com:
- Prevention = Annual Dental Screening, Colorectal Screening, Mammogram and/or Pap Smear
- Health Management = Annual Eye Exam and /or Annual Physical Exam
Complete your preventive exams
IMPORTANT:When you schedule your annual exams, be sure to tell your provider to code these exams as a “Wellness Screening” so they are 100% covered by your Concordia Health Plan. Any follow-up appointments related to diagnostic testing, acute issues, etc. may not be reported as “Wellness,” so discuss this with your provider.
