The Premium Plus option combines Medicare coverage with active employee Concordia Health Plan coverage. Not only do you get to keep the health plan's benefits such as the Vitality Wellness Program and vision benefits, you receive comprehensive medical and prescription coverage through Medicare.
Premium Plus provides enrolled workers with coverage for medical, vision, dental, hearing, and prescription drugs. Members can also participate in Vitality, have access to the Employee Assistance Program, receive Medicare Part B premium reimbursement and can utilize Teladoc Diabetes Management and Wondr weight loss programs.
Your employer must have fewer than 20 full and part-time employees on payroll to qualify. Other restrictions apply.
If your employer is eligible, you and/or your enrolled spouse must be 65 or older and an active LCMS worker to qualify for this opportunity. Other restrictions apply.
How Do I Apply?
Prior to you or your spouse turning 65, you will receive information from Concordia Plan Services about the Premium Plus Option.
