Preventive Care

Covering preventive care at 100%

Prevention is the best path to lifelong health. Annual wellness visits can catch problems early — when they’re easier to treat. The Concordia Health Plan covers you and your family 100% for preventive care so you can be proactive with your health, leading to a healthier you and a healthier family.

There’s a financial benefit to preventive care, too — you don't pay anything, no copays or deductibles. If you need additional health care because of something detected during a preventive care exam, your Concordia Plan Services benefits have got you covered.

Be sure to say at your preventive visit check-in: “This is my annual wellness visit. I’d like it billed as preventive only.”

Preventive care benefits include:

  • Annual wellness exam with your primary care physician.
  • Women’s health – mammogram, pap smear, osteoporosis screening.*
  • Colon cancer screening.*
  • Prostate exam.*
  • Well-child exams.
  • Immunizations.
  • Cholesterol/lipid profile.
  • Diabetes screening.

*Subject to certain age requirements
