Your Benefits Enrollment Checklist
Whether you’re a new worker, participating in open enrollment or updating your benefits, we’ve made enrollment easy. Use the Benefits Enrollment Checklist below to help you complete the process and take full advantage of the benefits your ministry is providing.
A pension is income you’ll receive from the Concordia Retirement Plan after you have retired. If you’re eligible, your ministry has already enrolled you and is paying the full cost of the benefit. No action from you is required for enrollment. Once you’ve reached the minimum years of service to become vested, you’ll receive a monthly income benefit in retirement. It’s your paycheck for life!
Personal savings is a critical component of your retirement plan. The 403(b), if you’re eligible, supplements your pension benefits to help you cover expenses in retirement. Any amount that you save — whether $10 or 10% of salary — will make a difference and you can adjust your savings rate at any time. Our 403(b) offers tax advantages, flexibility and low fees. Your ministry is providing disability and life benefits through the Concordia Disability and Survivor Plan. If eligible, your ministry will enroll you in the plan and pay 100% of the cost. No action from you is required during your enrollment.
Disability benefits will provide you with income replacement when you miss work due to a medically necessary reason, like a scheduled surgery, maternity leave, illness or injury.
If your employer is providing you with Basic Life Insurance, review your current coverage and use the life insurance calculator to help decide if you need to enroll in Supplemental Life Insurance or other voluntary insurance benefits.
If you want an increased level of life insurance, you have a unique opportunity when you are newly hired to purchase Supplemental Life Insurance for you or your spouse WITHOUT providing any medical records. You are eligible for up to $200,000 and your spouse for up to $25,000 without evidence of insurability (EOI).
Enrolling your dependents and designating your beneficiaries is an important step that is often overlooked.
Health Plan: Select dependents for the Concordia Health Plan, if eligible, on the Member Portal.
HSA: Beneficiaries for your health savings account must be designated separately through your health savings account provider (e.g., HealthEquity's website).
Life insurance: Select dependents for your Basic Life and Supplemental Life insurance benefits on the Member Portal.
Student dependent eligibility. For the Concordia Disability and Survivor Plan, you will need to confirm if you have any child dependents age 21 or over who are both unmarried and full-time students.

Register with Quantum, your personal healthcare navigator. Your go–to resource for any medical or prescription questions, including ID cards, deductible and claims status, or getting guidance with complicated healthcare issues.
Register with your telehealth provider, your anytime doctor. You don’t always have to go to the doctor because the doctor can virtually come to you. Conveniently access physicians, psychologists, nutritionists, dermatologists or get an expert second opinion even if you need help in the middle of the night or on a weekend.
Register for Evernorth Confide, your behavioral health navigator. You have expanded mental healthcare solutions to address what is challenging your mental well–being, which you can access in–person or digitally. Find personalized care through assessment and care navigators.
