Providing your employees and their families with the safety net of life insurance helps provides peace of mind. In addition, life insurance is increasingly becoming a standard of quality benefits at employers.

Workers who are enrolled in the Concordia Disability & Survivor Plan receive a Basic Life benefit for the worker and their enrolled dependents. The amount of the benefit is determined by the individual worker’s circumstances.

This benefit is funded fully by the employer.


Reporting a Death

To report the death of a worker or dependent, call Concordia Plan Services at 888-927-7526. We are here to help walk the worker (or the surviving beneficiaries) through the claim process.

Once we have received your call, we will mail you a packet with resources and the necessary paperwork. Please note that CPS will need a certified copy of the death certificate.  

In the event that a member dies while overseas, the death needs to be reported to the U.S. Embassy in the country in which the death occurred. Additional documentation needed may include: police report, physician’s statement, certified death certificate, and cancelled passport.

Click Your Beneficiaries to review and update beneficiaries.

Will Preparation Services

Active workers enrolled in the Concordia Disability and Survivor Plan can receive legal will preparation assistance from Securian, including will creation, financial calculators and other financial resources and legal information.
