You spoke, we listened and have taken action to provide an improved overall experience for you and your workers. Starting May 1, Sedgwick Disability Services will administer disability benefits under the Concordia Disability and Survivor Plan (CDSP).
Here’s what you and your workers can expect with Sedgwick:
Employers can report a claim on a worker’s behalf by calling Sedgwick at 888-550-1617.
Streamlined claims experience
Faster processing times for claims.
Greater visibility and transparency
Workers can track their claim status and chat directly with Sedgwick through the MySedgwick portal in addition to emailing and/or calling Sedgwick directly.
Employers will receive status notifications throughout the claim process.
Continued support from Concordia Plans
After a claim is submitted to Sedgwick, the CPS Health & Wellness Team will reach out directly to workers with additional support and guidance.
What’s happening with current claims?
If a member is currently on short-term disability (disability expected to last 26 weeks or less), they will continue their claim with Lincoln Financial. If their condition extends beyond 26 weeks and they move to long-term disability, the claim will transition to Sedgwick.
Members currently on long-term disability (disability has lasted more than 26 weeks) were transitioned to Sedgwick from Lincoln Financial on May 1. These members have been notified and were asked to update their direct deposit information with Sedgwick to ensure uninterrupted payments.

Invoice payment address changing June 4
Starting June 4, there will be a new address for invoice payments sent via U.S. postal mail. This address should only be used for invoice payments.

Q: How much disability and life insurance coverage do I have through the CDSP?
Members can log in to their Member Portal from ConcordiaPlans.org and open their Personal Statement of Benefits, where they’ll find coverage amounts listed in the “Disability Benefits and Survivor Benefits” sections.
Q: Can I continue my CDSP coverage after I leave my employer?
Members can convert their CDSP life insurance coverage into an individual life insurance policy when they leave their employer.
Workers filing a new disability claim after May 1 will submit their claim directly to Sedgwick by calling 888-550-1617 or online through their portal: mySedgwick.com/ConcordiaPlans.
