Renewal Notices: Mailing in mid-June
Employer Benefits Election Period: Sept. 2–12
Member Open Enrollment: Nov. 3–14
Timely action during these windows helps ensure smooth transitions and continued access to high-quality, affordable benefits. Please plan to complete your employer elections by Sept. 12. We are unable to extend the deadline beyond this date.
Update your systems with our new
invoice payment address
Starting TODAY (June 4), we have a new address for invoice payments sent via the U.S. Postal Service. This address should only be used for invoice payments.

It’s official! We’ve now transitioned to Sedgwick for administration of Concordia Disability and Survivor Plan disability benefits. Sedgwick is a global leader in claims administration. They focus on getting people back to wellness, reviving communities and helping organizations succeed.
Workers filing a new disability claim should contact Sedgwick directly at 888-550-1617 or through the Sedgwick portal: mySedgwick.com/ConcordiaPlans.The new portal provides workers with greater visibility into their claim status and offers a more convenient way to communicate with their case manager through enhanced, user-friendly features.
Disability benefits provide income replacement when a worker needs time off due to a medically necessary reason, like a scheduled surgery, maternity leave, illness or injury. Your workers can see their estimated disability benefits by logging in to the Member Portal and viewing their Personal Statement of Benefits in the Communication Center.
