Through September 30, 2021, the CHP is covering testing and treatment for COVID-19, as well as telehealth services* at 100% - this means no out of pocket costs to you. By not having this burden of cost, we hope that you will be able
to get the care you need during this difficult time.
Testing for COVID-19. Testing is covered 100% through members’ CHP benefits, for both in-network and out-of-network providers, when testing is medically necessary: when members are referred by a medical professional, have been exposed to someone or are having symptoms. Please note that tests as a condition of employment are not covered at 100% by the Plan. Members
should contact their employer about free testing services if their job status is dependent on taking a test. If the test is designated as work-related, the member will be subject to normal costs like deductible, copay and coinsurance.
Treatment of COVID-19. If you’re diagnosed with the coronavirus disease, treatment is covered 100% ONLY for in-network providers. While testing and treatment for COVID-19 continues to evolve, coverage will include
All FDA-approved testing methods.
Inpatient and outpatient services.
Durable medical equipment.
Skilled care needs.
FDA-approved drugs when they become available.
Telehealth services*. So you don’t have to step foot in a doctor’s office, an urgent care or an emergency room and can remain in the security of your home during this time, we’re providing 100% coverage
for all telehealth visits. This includes visits for any common illness, such as flu, stress/anxiety, emotional support, skin conditions, allergies, etc. This applies to all telehealth benefits offered through each of the CHP options.
*Through CPS-approved telehealth vendors. Meetings with your doctors through Zoom or other video chat services are covered with your regular cost-sharing and deductibles.
The COVID-19 vaccine itself will initially be provided by the government at no costs to health plans or members. The administration of the provider dosing the vaccine is a cost that will be covered under the Concordia Health Plan at 100% with no cost sharing with the member.
Early on, the COVID-19 vaccine supply will be limited, with the first doses designated for frontline health care workers and residents of long-term care facilities. Over the first three to four months of 2021, we anticipate that vaccine supply will increase significantly and there will be opportunities for additional groups to be vaccinated. Distribution will vary across each state, as state governments are responsible for the distribution to their population.

All COVID-19 related services, including testing and treatment, are covered at 100% in the Concordia Health Plan with no member out-of-pocket costs*. Members served by Blue Cross Blue Shield of Minnesota, Cigna and UMR can be tested at any facility, even those out-of-network, for no cost. Kaiser Permanente members must be tested at a Kaiser network facility.

All telehealth visits for any reason (including non-COVID) are covered at 100 percent without copays or deductibles, including for members in high deductible plans, until September 30, 2021. This does not include Zoom or other video conference appointments

Yes. Direct deposit is a payment option in which your monthly benefit payment is automatically deposited into your bank account on the first business day of every month. To begin, simply fill out the Electronic Funds Transfer (EFT) form and return the completed form to our office. If you are unable to return the completed form with a voided check, please contact your bank to confirm your correct routing number and account number. Upon receipt of your completed form, our office will process your change to direct deposit and will send you a confirmation letter.
