With this tool, you can seek treatment for a host of common illnesses, such as colds, flu and more, all without leaving the house. You will also save money because you’ll only pay a minimal copay.* This is just one way Concordia Health Plan options put you in control of how you spend your healthcare dollars.
*If you are enrolled in a high deductible option, the charge will be applied to your deductible or coinsurance.
Answers for Common Questions
Blue Cross Blue Shield and Cigna members can check out the 24-hour Nurse Line feature, which gives you the ability to call a registered nurse with medical-related questions and concerns, 24/7. RNs will provide guidance and support for members when making decisions about their health. For instance, consider calling the 24-hour Nurse Line first at 800-605-6621 if you have questions about an upcoming procedure or if your child is showing symptoms that you think could be Chicken Pox.
Quality programs that encourage preventive wellness or those designed to address specific at-risk conditions can prove to be beneficial to your overall well-being and your wallet. CTHT coaches are dedicated to working collaboratively with and specifically for CHP members. Coaches understand all the health-and-wellness resources available to you and know how to best leverage those resources. Vitality and Omada
Your support team, be it CTHT or your Kaiser Permanente physicians, could also direct you to two other health and wellness resources offered through you’re the Concordia Health Plan – Vitality and Omada.
Vitality is a health-and-wellness program that integrates healthy activities and choices into daily life and provides personalized feedback. Members can set health-and-wellness goals, put those goals into action and earn points for doing so. In addition, members receive a $100 subsidy for a wearable device (e.g., Fitbit) and could be eligible for a gym membership rebate of up to a $250.
The Omada program, available right now to Blue Cross Blue Shield members but open to all Concordia Health Plan choices as of Jan. 1, is designed to help participants lose weight, improve their health and avoid the risks of chronic disease — one small step at a time. Members and their dependent(s) age 18 or older at risk of type 2 diabetes or heart disease can apply for Omada. If determined eligible, participants will begin a 16-week journey to well-being with a:
- Full-time health coach
- Wireless smart scale
- Online peer group
- Program that adapts to the member

- Provide up to six free face-to-face sessions per issue per year with an EAP professional
