Blue
Cross Blue Shield has set up three ways for you to submit
your medical claims:
Claims
may be submitted using a Blue Cross Blue Shield Medical
Claim Form.
It is
important that you include itemized billings for the medical
services you receive. The bills should include a diagnosis,
date of service, type of service, amount of service, provider
name and type of provider. Also, it would be helpful if you
could translate the language to English and the foreign currency
into U.S. dollars, using the exchange rate in effect at the
time services were provided. To find the exchange rate, logon
to www.oanda.com/cgi/convert/cheatsheet.
All claims must be filed within one year of the date of service.
Remember
to provide the World Mission department with a current mailing
address in the U.S. where all explanation of benefits (EOBs)
can be sent. |
