Using CHP's PPO Program

Concordia Health Plan has contracted with Blue Cross Blue Shield of Minnesota to administer the medical claim benefits available under our Preferred Provider Organization (PPO) program. Blue Cross Blue Shield Plans maintain networks of hospitals and other medical providers in almost every state in the U.S. There is also a worldwide network of hospitals located in various countries around the globe.

Anytime you can access care through network (participating) providers, network benefits are payable. Your out-of-pocket cost will be less and you’ll never need to file claim forms. Blue Cross Blue Shield allows access to all of their PPO networks, regardless of location, through the BlueCard PPO program. Your Blue Cross Blue Shield ID card is your link to obtaining your network benefits wherever you may travel or live. Always carry it with you.
