you need to take Part B when you first become eligible because
of our non-duplication of benefits provision. If you have

your coverage in the Concordia Health Plan (CHP) is currently
through active full-time work/employment, the CHP is the primary
payer and Medicare is secondary. However, if your coverage
is not through current active full-time
employment, meaning your status with the CHP is disabled,
retired, or on a CHP extension, then Medicare is the primary
payer and the CHP is secondary.

in Medicare due to end-stage renal disease (ESRD). Who is the
primary payer? |

CHP is the primary payer for the first 30 months
that you qualify for Medicare because of ESRD. Medicare becomes
the primary payer beginning with the 31st month
and the CHP is secondary.

should contact Medicare directly at 1-800-MEDICARE (1-800-633-4227).
Please make sure when you speak with Medicare that they clearly
understand whether your CHP coverage is through current
active work/employment. After speaking with
Medicare, please contact us at 1-888-927-7526, ext. 6870 to
inform us as to what was determined by Medicare.
