Purpose of this policy
The purpose of this policy is to explain the types of information Concordia Plan Services (CPS) may gather about your dependent(s) or you, how we may use that information, whether we disclose it to anyone, and the choices you have regarding our use of, and your ability to change or correct, the information.
CPS gathers and maintains a wide variety of financial, health, and personal information, which is stored in electronic and hard copy files. This includes personal details, health, and financial information concerning our employers, their employees, and dependent(s) of employees.
Although some of this information can be located in public records, other information is proprietary, sensitive, and/or confidential. CPS is committed to safeguarding your dependent(s)’ and your personal information.
Safeguarding your personal information
Some pages on our website request and collect information that personally identifies your dependent(s) or you or allows us to contact you personally (for example, your email address). Information collected by CPS about your dependent(s) or you from our website may be tabulated for statistical purposes and those results may be provided to others for plan administration purposes, but your dependent(s) or you will never be individually identified in those results. The data may be used to inform you of events and activities in which we think your dependent(s) or you may have an interest, or opportunities to support the work of CPS. We may also use the data to evaluate our benefit programs and design future offerings. Although the information collected is used primarily for purposes specified on the submission pages, we may add that information to our records and use it to enhance our future contact with you.
Although we may provide aggregate data for the purposes of reporting and/or compliance with government requirements, your dependent(s)’ and your personal information will not be identifiable in such data. CPS will not share any of your dependent(s)’ or your personal information with any third parties, except as required by law or for Concordia Plan purposes with our third party administrators or vendors. We do not rent, sell, or lend mailing lists, telephone numbers, mailing addresses, or email addresses to other individuals or organizations outside of CPS or The Lutheran Church—Missouri Synod (LCMS). We strive to keep the private personal information of your dependent(s) and you as private information, but you agree to hold us harmless for any inadvertent disclosure and for any disclosure to our third party administrators or vendors for Concordia Plan purposes.

We take no responsibility for, and exercise no control over, the organizations, views, or accuracy of the information contained on other websites.
