Enrolling Workers

HSA Enrollment - 

Print the following forms, and have the worker complete and return to you. Return the forms to SelectAccount via the website or by mail.

Members enrolled in Health Wise Plus 3000, a CHP option administered by Cigna, should set up an HSA with SelectAccount in order to earn incentives (deposits into their account) from the CHP. 


HRA Enrollment -

Once you have returned your Plan Design Guide (PDG), the rest of the process is automated - there are no extra forms to complete. 

FSA Enrollment - 

Print the FSA enrollment packet and have the worker complete and return to you. Return the forms to SelectAccount via the website or by mail.

Submitting a Manual Claim

For services that are not automatically reimbursed by your HSA or FSA through Crossover or debit cards, you can file a manual claim with SelectAccount. Complete a form and submit the appropriate documentation.
