| If the
enrollment form is received by Concordia Plan Services more
than 60 days after the worker’s full-time employment date,
the worker will be enrolled in: |

Late
enrollment in CHP – only
during an Annual Open Enrollment
period, unless the reason for the late application falls under
Special Enrollment
eligibility requirements as defined by the Health Insurance
Portability and Accountability Act (HIPAA). If an employer
offers its workers the option of another health plan in addition
to the CHP, any worker who joins the other health plan and
later wants to switch to the CHP will be eligible for enrollment
in the CHP only during an Annual
Open Enrollment or Special
Enrollment period. |

Late
enrollment in CRP – retroactive to the
first day of the month following the date of the worker’s
full-time employment. Contributions will be billed retroactively,
and a late payment fee will be assessed to help cover lost
investment earnings. |

Late
enrollment in CDSP – effective the first
day of the month following the receipt of the enrollment form
in the Concordia Plan Services office. |
