Readoption—Eligibility
Criteria
An employer
that has previously withdrawn from the Concordia Health Plan
(CHP) may apply for re-adoption of that plan by completing
a new adoption form. However, upon readoption, there will
be certain provisions and underwriting requirements that will
need to be satisfied before the readoption agreement is approved.
Those provisions and underwriting requirements are: |
- A
period of three years must have elapsed since the withdrawal
occurred;
- Employer
was in good financial standing at time of the withdrawal;
- Certain
minimal participation requirements are satisfied.
- For
workers enrolling in the CHP at the time the employer is
readopting the Plan, there will be no CHP coverage for any
pre-existing condition for
the first 12 months of membership for that worker or any
dependents. However, the pre-existing condition limitation
period can be eliminated or reduced if the worker or his/her
dependents had prior health coverage immediately prior to
CHP enrollment and a HIPAA certificate is provided to document
this other coverage. (The pre-existing condition limitation
does not apply to prescription drugs, dental care, or the
Member Assistance Program. Also, the limitation does not
apply toward pregnancy-related expenses.)

After
a readoption request is received, it will be reviewed by the
Concordia Plan Services staff. The employer will then be notified,
in writing, if the readoption application has been approved
and the effective date of coverage. |

| An employer

and Disability and Survivor Plan (CRP/CDSP) may apply for readoption
of those plans by completing a new adoption form. Upon re-adoption
in CRP/CDSP, a period of three years must have elapsed since
the withdrawal occurred. |
