Plans
Enrollment
The Concordia
Plans are employer-sponsored benefit programs. An employer
makes the decision whether to participate in the Plans for
its workers. The employer can adopt one of two benefit packages::
- The
Church's Plan which consists of the Concordia Health Plan,
Concordia Retirement Plan, Concordia Disability and Survivor
Plan, and Concordia Retirement Savings Plan,
- Package
3 which consists of the Concordia Retirement Plan and Concordia
Disability and Survivor Plan.

2, which consists of the Concordia Health Plan*, Concordia
Retirement Plan, and Concordia Disability and Survivor Plan,
is a grandfathered/closed group that is available only for employers
offering these three Concordia Plans prior to January 1, 2006.

| Package
4, which consists of the Concordia Health Plan*, is
also a grandfathered/closed group that is available for employers
offering only the Concordia Health Plan prior to July 1, 2005. |

An employer who offers the CHP in addition to another health
plan will not be able to participate in the CHP beginning January
1, 2007. |

| If
an employer has adopted the Concordia Retirement Plan
and the Concordia Disability and Survivor Plan,
all full-time workers, regardless of age, sex, occupation, or
faith, must be enrolled in these two Plans. |

| If
an employer participates in the Concordia Health Plan,
all full-time workers, regardless of age, sex, occupation,
or faith, must be offered the opportunity to enroll
themselves and any eligible dependents.

| Enrollment
in all Plans is effective the first day of the month following
the employment date. If employed on the first day of the month,
enrollment will be on the first day of the following month.
Provisions concerning late enrollment will apply to workers
who desire to enroll themselves or a dependent in the CHP after
the first date of eligibility. |

| A
husband and wife, who are both employed by the same or different
employers participating in the CHP, must each be enrolled separately
as workers if coverage for both is desired. A spouse cannot
be enrolled as a dependent if eligible to be enrolled as a worker.
This avoids the possible inequity of an employer paying for
the coverage of a spouse who is also employed by another LCMS
employer. Eligible dependent children may be enrolled by either
parent, but not both. |

| Late
enrollment of a worker (where the application is received more
than 60 days after the date of full-time employment) is subject
to special rules. CRP enrollment is effective retroactive to
the worker's original eligibility date, and retroactive contributions
plus interest will be due from the employer. CDSP enrollment
is effective the first day of the month following receipt of
the enrollment form by the Concordia Plan Services office. CHP
enrollment of the worker and any dependents is subject to special
enrollment requirements, and enrollment might not be available.

| Assigned
Worker Enrollment |
New
graduates of LCMS seminaries and colleges who are assigned
by the Synod’s Board of Assignments for employment with
your organization will normally be enrolled, as are other
workers, on the first day of the month after reporting for
work.
However,
if the worker has completed all academic requirements for
graduation, the plan provisions allow the employer to request
that an assigned worker be enrolled prior to actually beginning
employment. This earlier date can be the first day of any
month following the date of assignment, but no later than
the first day of the month after reporting for work at the
employer. |
