Concordia Health Plan Rates for Active Workers and Deployed Workers

Contributions are based on a geographical pricing system, which means that contribution rates reflect the cost of health care in the employer's local area. The amount is determined by the class of coverage selected, CHP option, and the ZIP Code area in which the employer is located. The cost for Deployed Workers is geographically priced using the ZIP code area in which the Deployed Worker lives. Five classes of coverage are displayed for each CHP option:

  • Self Only
  • Self and Spouse
  • Self and Child(ren) Only
  • Self, Spouse, and Child(ren)
  • Child Only

The contribution rates are determined based upon the class of coverage and whether the member or dependent is eligible for Medicare. The cost for the person not covered by Medicare is geographically priced (like active workers), using the ZIP Code area in which the member lives.
