The Concordia Plans are employer-sponsored benefit programs. An employer makes the decision whether to participate in the Plans for its workers. The employer can adopt one of two benefit packages:
- The Church's Plan which consists of the Concordia Health Plan, Concordia Retirement Plan, Concordia Disability and Survivor Plan, and Concordia Retirement Savings Plan.
- Package 3 which consists of the Concordia Retirement Plan and Concordia Disability and Survivor Plan.
Package 2, which consists of the Concordia Health Plan*, Concordia Retirement Plan, and Concordia Disability and Survivor Plan, is a grandfathered/closed group that is available only for employers offering these three Concordia Plans prior to January 1, 2006.
Package 4, which consists of the Concordia Health Plan*, is also a grandfathered/closed group that is available for employers offering only the Concordia Health Plan prior to July 1, 2005.
* An employer who offers the CHP in addition to another health plan will not be able to participate in the CHP beginning January 1, 2007.
If an employer has adopted the Concordia Retirement Plan and the Concordia Disability and Survivor Plan, all full-time workers, regardless of age, sex, occupation, or faith, must be enrolled in these two Plans.
If an employer participates in the Concordia Health Plan, all full-time workers, regardless of age, sex, occupation, or faith, must be offered the opportunity to enroll themselves and any eligible dependents.
Enrollment in all Plans is effective the first day of the month following the employment date. If employed on the first day of the month, enrollment will be on the first day of the following month. Provisions concerning late enrollment will apply to workers who desire to enroll themselves or a dependent in the CHP after the first date of eligibility.
