The Concordia Health Plan Be Well Rewards program was introduced in 2011 as an extra incentive to help Concordia Health Plan members make healthy choices.
Because we want to help members stay on track and make healthy habits an important part of their daily routine, the Be Well Rewards program will continue for 2014.
Year three also offers an expanded menu of healthy activities and wellness programs and up to $250 in rewards! And, once again, employers will earn a dollar-for-dollar match on what their enrolled workers and their enrolled spouses earn by participating in these healthy activities.
Accessing the Be Well Rewards Program has changed slightly from past years. You will no longer visit www.BeWellServeWell.com, and instead log into www.myCPS.org to access the Be Well Rewards Program. For instructions on how to do so, view this tutorial.
