Cancer Support Program: 800-605-6621 (Not available for Medicare Members)
This support program offers resources to support CHP members whose lives may be impacted in various ways by cancer. The program supports members and their families throughout the different stages of this disease, including prevention strategies, treatment, and survivorship planning.
Prevention Strategies: CareAllies works with members to promote lifestyle changes that reduce the risk of developing cancer, including appropriate risk screenings and follow up, targeted mailings, and outreach to members to help them adhere to recommended screenings.
Treatment: CareAllies’ clinicians will help identify facilities and physicians with the best outcomes for treating a member’s specific disease and assist members in accessing the appropriate care.
Survivorship Planning: This end-of-life component of the program focuses on supporting members and their families as they transition from active treatment to hospice or palliative care. The specialists in this area help families plan for the member’s passing and encourage members to articulate their desires and choices. In addition to helping members access the appropriate hospice providers, they also provide emotional and clinical support to families planning this care.

This secure, online questionnaire will provide health plan members with an assessment of their health. Once the results are calculated, they can be printed and discussed with their physician during the next visit.
The private health information will not be shared with employers, circuits, or districts, nor will it be used to determine contribution rates or eligibility for benefits.
An assessment that indicates a health concern (i.e., high blood pressure, high cholesterol, etc.) will trigger personal contact either by phone or by mail from a CareAllies Health Advisor (see details below). The Health Advisor is a medical professional – a health coach – who can offer information about disease prevention, condition management, and lifestyle modification.

Health Advisor Program: 800-605-6621
Health Advisors (Certified Health Education Specialists, Nurses, Behaviorists, or Nutritionists) will help those church workers with health risks develop a personal action plan to reduce those risks. These are caring professionals who want to make a difference in the lives of church professionals.
The Health Advisors will also provide ongoing support so workers can sustain their new positive health practices. This program is similar to having a personal coach to provide support, advice, and encouragement whenever it’s needed. Through phone conversations, a supportive partnership is built between the participant and the advisor to help the participant achieve his/her health-related goals.
The Health Advisors don’t replace physicians—quite the opposite. Health Advisors help members engage in more meaningful discussions with their physicians by providing members with information needed to make more informed healthcare decisions. The Health Advisors, in a sense, serve as guides to help members navigate through the healthcare system.
How it worksIf, after taking the HA, a worker is identified as being at risk for a particular condition, a Health Advisor will contact the worker by telephone to introduce the program and start establishing a course of action to take control of the situation so the condition doesn’t become worse. Healthy Pregnancies, Healthy Babies: 800-605-6621 (Not available for Medicare Members)
Concordia Health Plan (CHP) members, including enrolled dependents, who are expecting a baby, are encouraged to participate in the CareAllies Healthy Pregnancies, Healthy Babies program. The program is designed to:
- improve the quality of maternity care through comprehensive clinical assessment of pregnant members;
- develop care management plans tailored to each member’s specific needs; and
- deliver improved education and tools for self-care.
CareAllies clinicians conduct initial and follow-up screenings for all pregnant members to assess for specific medical conditions that will impact their pregnancy (such as diabetes) or who have a history of a complicated pregnancy. Ongoing management services screen for the possibility of new problems developing during pregnancy such as gestational diabetes or premature labor. The post-partum assessment includes screening for depression and counseling on newborn issues.
CIGNA also offers a cash incentive to encourage CHP members to participate in the Healthy Pregnancies, Healthy Babies program. Members who enroll by the end of their first trimester will receive $150; members who enroll by the end of their second trimester will receive $75. Cash incentives will be distributed upon completion of the program. Only one cash incentive is offered per pregnancy.

Concordia Health Plan members who choose to make a change regarding weight loss, smoking cessation, and stress management can call CareAllies at 1-800-605-6621 to get in touch with a personal coach who will help set up a program to achieve success in these areas. - For weight loss, 47% achieve greater than one unit drop in BMI (body mass index), and 85% of those who achieve a 5-10% weight loss at 6 months maintain it at 12 months.
- For hypertension, 63% of participants report a decline in stress.
- For tobacco cessation, the quit rate averages 27%, nearly twice the industry standard of 12-20%.

This program provides those Concordia Health Plan members with chronic health conditions a plan to help manage their condition. Members receive personal assistance from experienced and caring registered nurses and other clinicians. The clinicians provide support, help develop actions plans, and reinforce behavior changes to help members monitor and manage their condition. By following the managed plan, members learn to anticipate their symptoms and better manage them to reduce the risk of complications. The disease management program helps members with a chronic condition spend less time being a patient and more time leading their lives and ministries.
