Select 500 & 1000, and Options A-D are preferred provider organization (PPO) managed care programs offering network and out-of-network benefits. Each option varies by copay, deductible, and coinsurance amounts. Network deductibles range from $0 to $1000 for individual (self only) coverage. Preventive care is covered if performed by a network provider, with well-child care covered at 100% (subject to specific age and frequency limits).
Out-of-network care is available but is subject to a higher deductible, coinsurance, and out-of-pocket maximum. Preventive care is not covered when performed by a out-of-network provider.
Choice 1500, 2000, and Option HDHP, our high deductible health plan options, are designed to work in conjunction with a health savings account (HSA) or health reimbursement arrangement (HRA). They are managed care programs offering both network and out-of-network benefits. Network preventive care is not subject to the deductible and is covered at 100%.
Out-of-network care is available but is subject to coinsurance as well as a higher deductible and out-of-pocket maximum.
Option E, our CDHP option, is designed to work in conjunction with a health reimbursement arrangement (HRA). It is a managed care program offering both network and out-of-network benefits and represents a hybrid plan design, with characteristics of Options A-D and Option HDHP.
