This plan provides coverage for active workers enrolled through a participating LCMS organization and individual participants (such as early retirees, surviving dependents, disabled members, and persons electing an extension of coverage) who are under the age of 65. (Individual participants who are age 65 and older may be eligible for the Concordia Health Plan for Medicare Members, our Medicare supplemental coverage option.)

The Concordia Health Plan provides benefits for a wide range of healthcare needs. Benefits are bundled together under the health plan so employers are certain to provide comprehensive coverage for their workers. The plan consists of:

The CHP uses Blue Cross Blue Shield of Minnesota and Cigna Healthcare to administer its medical benefits. CHP Options differ by their level of benefits and type of plan design.

Each year LCMS employers have the opportunity to designate the Concordia Health Plan option(s) to provide for their workers. Concordia Plan Services strives to provide options that best balance the need for comprehensive benefits with the mindfulness of organizational budgets.

PPO Options

Options A-D are Preferred Provider Organization managed care programs offering network and non-network benefits. Each Option varies by copay, deductible, and coinsurance amounts. Network deductibles range from $0 to $1000 for individual (self only) coverage. Preventive care is covered if performed by a network provider, with well-child care covered at 100% (subject to specific age and frequency limits).

Non-network care is available but is subject to a higher deductible, coinsurance, and out-of-pocket maximum. Preventive care is not covered when performed by a non-network provider.


CDHP Options

Option HDHP, our high deductible health plan option, is designed to work in conjunction with a Health Savings Account (HSA) or Health Reimbursement Arrangement (HRA). It is a managed care program offering both network and non-network benefits. The network deductible is $2,850 for individual (self-only) coverage, with 0% coinsurance for the CHP member for medical care after the deductible has been satisfied. The deductible applies to most medical care (medical services, hospitalization, mental health care, substance abuse care, prescriptions, etc.). Network preventive care is not subject to the deductible and is covered at 100%.

Non-network care is available but is subject to coinsurance as well as a higher deductible and out-of-pocket maximum.

Option E, our consumer-directed health plan option, is designed to work in conjunction with a Health Reimbursement Arrangement (HRA). It is a managed care program offering both network and non-network benefits and represents a hybrid plan design, with characteristics of Options A-D and Option HDHP.

The Option E network deductible is $1,800 for individual (self-only) coverage. After the deductible is satisfied, services provided in a physician’s office are covered at 100% (no copays or coinsurance), while other medical and hospital services require 20% coinsurance for the CHP member. Unlike Option HDHP, prescription drugs and mental health or substance abuse care are not subject to the medical deductible. Similar to PPO Options A-D, medication can be purchased for an applicable copay, and mental health and substance abuse care has separate coinsurance and copays for benefits. Preventive care is not subject to the deductible and is covered at 100%.


HMO Options

HMO Options are available in southern California and select locations across the country. To find out if an HMO option is available in your area, please contact us at 888-927-7526. HMO options offers network benefits only (care must be accessed through a participating provider). Most services are available for a copay. There is no medical deductible and most services carry 0% coinsurance for the CHP member. There is no non-network benefit except for Ambulance charges for true emergencies.
