How To Join The Church’s Plan

To participate in The Church’s Plan, LCMS employers must do the following:

• meet Minimum Participation Requirements in the Concordia Health Plan.
• complete a Joinder Agreement to join The Church’s Plan.
  

When an employer joins The Church’s Plan, enrollment will be effective the first day of the month following receipt of the Joinder Agreement.
